Bells Web App Release - 2024.05.02

Summary

  • For myAvatar users, added a new Validation to check against a patient's authorizations.
  • Added a new custom note section question type for Decision Only.

  • Assorted bug fixes

myAvatar Authorizations

  • For myAvatar users, added a new Validation to check against a patient's authorizations. The new validation can be configured based on services to check against a minimum number of authorizations for the patient. This update requires myAvatar patch - Avatar CareFabric 2024 Update 27.

Custom Note Sections

  • Added a new custom note section question type for Decision Only. This new question type is available to add in any custom note section type and allows users to make a selection to enable additional questions in the note without showing on the output. 
  • For myAvatar and myEvolv users, updated the import functionality as part of custom note sections to allow users to import section headers into the section. As part of this update, section headers (when available) will auto-import into the section when select all is used. 

 

  • Corrected a bug as part of drop-ins incorrectly sorting them in the list. 
  • For myEvolv users, fixed an issue as part of sub-forms preventing custom note sections from being saved when there is no header. 
  • For myEvolv users, updated group notes to respect the optional place of service organization preference. 
  • Made a variety of fixes as part of scribbles, including copy/paste into a note, highlighting functionality and styling.


 


Bells Web App Release - 2024.04.25

Summary

  • For myAvatar users, we're introducing a Support Documentation note that will support the Independent Note workflow in myAvatar.
  • Added the ability to link custom note section long text question types to a form or narrative custom note section. 
  • Updated the patient chart display with a new tab for Treatment Plan. 
  • Added a new organization preference to the Configurations tab for displaying the sync issues on the user dashboard.
  • Updated the label substitutions on the Compliance tab with a new label substitution for Practitioner. 

Support Documentation Note

  • For myAvatar users, we're introducing a Support Documentation note that will support the Independent Note workflow in myAvatar. This new type of note allows users to enter simplified notes that only need the Patient's enrollment, program and myAvatar Note Type to complete the note. Added a new organization preference to determine the default setting for when starting a note between the standard start note or starting a Documentation note. The start note button has been updated to include a dropdown menu to select between the two types of notes. 

Custom Note Sections

  • Added the ability to link custom note section long text question types to a form or narrative custom note section. When used in a note, the long text field will be replaced with the form or narrative custom note section selected int he configuration. 

Patient Chart

  • Updated the patient chart display with a new tab for Treatment Plan. The new tab shows the treatment plan name and date along with it's goals, objectives, interventions and problem (where applicable).

Organization Preference 

  • Added a new organization preference to the Configurations tab for displaying the sync issues on the user dashboard. When set to on, the Sync Issues widget would show on the right hand side under Unsigned Notes if the user has sync issues to show.
  • Updated the label substitutions on the Compliance tab with a new label substitution for Practitioner. This label will update throughout the web application where Practitioner, Staff and some instances of User were displayed. This new label substitution will also be updated in the mobile application on it's next update. 

 


 


Bells Web App Release - 2024.04.18

Summary

  • Added new filtering capability based on the show status of the note.
  • For myAvatar users, made an update to the Note Type editor to no longer hard stop a user from saving a note type.
  • Updated the toolbar for text fields to include a new icon for drop-ins. 
  • For myAvatar users, made a change for group notes and appointments. 
  • Assorted bug fixes. 

Custom Note Sections

  • Added new filtering capability based on the show status of the note. Entire custom note sections can be filtered from the note based on the show status of the note. 

Note Types

  • For myAvatar users, made an update to the Note Type editor to no longer hard stop a user from saving a note type when the note doesn't include a custom note section Narrative or note type section Text. Moving forward it will be a warning to the user that can be bypassed. 

Drop-Ins

  • Updated the toolbar for text fields to include a new icon for drop-ins. With this update, we've changed the icon used for drop-ins in the toolbar and command menu. The same icon will be used in the mobile application on it's next release update. 

myAvatar Group Appointment

  • For myAvatar users, made a change for group notes and appointments. When a patient is added/removed from the group session when started from an appointment, Bells will make a call to myAvatar to add/remove the patient(s) from the appointment. If for some reason the update is unable to be made, Bells will alert the user.  

 

  • Corrected a bug in the Span Midnight validation preventing the configuration of not allowing to span midnight for all services. 
  • Fixed a bug displaying a user's last signed in date incorrectly in the user management display. 


 


Bells Web App Release - 2024.04.11

Summary

  • Updated the Redundant Much clinical recommendation to replicate the alternatives provided as new rules. 

Clinical Recommendations

  • Updated the Redundant Much clinical recommendation to automatically create the alternatives added to the rule as additional rules with the same configuration. For example, if I set up a Redundant Much rule for Observe with the alternatives of Watch, See and Witness, when I save, three additional rules for Watch, See and Witness will also be created with the same rules and alternatives. 

 


Bells Web App Release - 2024.04.04

Summary

  • Introducing a new way for Administrators and Supervisors to view data metrics in Bells.
  • For myAvatar and myEvolv users, added a new Custom Note Section type for Imports.
  • Enhanced custom note section settings, import display and question types available based on the Custom Note Section type. 
  • Updated Group Note Administrator capabilities to include the ability to edit individual patient notes in the group.
  • Changed the Configure Group Note display to show the services when the filter toggle is enabled. 
  • Assorted bug fixes. 

Administrator and Supervisor Dashboard

  • Introducing a new way to view data metrics in Bells with a new comparison view on the Administrator (Programs) and Supervisor (Individuals) Dashboard. The new view allows administrators and supervisors in Bells to compare up to 5 programs/users across 10 different metrics. In this view you also get to customize what metrics/widgets show and where they show in the display. 

Custom Note Sections

  • For myAvatar and myEvolv users, added a new Custom Note Section type for Imports. Import custom note sections are sections that include at least one question that has been imported from the EHR. Bells has taken care of migrating your existing Form type custom note sections that meet the Import type criteria to an Import type. If you have Narrative type custom note sections, those have not changed and will continue to be Narrative format. Moving forward, you can create new Import custom note sections.
  • Enhanced the custom note section setting for Include in EHR Note Narrative, what shows in the Import display as well as what types of questions can be added to the section based on the type of Custom Note Section being configured.

Group Notes

  • Updated Group Note Administrator capabilities to include the ability to edit individual patient notes in the group. This is triggered by a new organization preference called “Group Note Edit”. Even if the group administrator is allowed to edit, the note author must still sign the note. 
  • Made a change to the Configure Group Note display to show the services that will be available to select when the Filter services toggle is enabled. 

 

  • Corrected a bug for CT|One users that stopped showing the user credentials on signatures on the note in CT|One. 
  • Fixed a bug when an appointment is schedule for a no longer valid group preventing the user from starting the note.

Summary

  • We've redesigned the Note Type Editor display!
  • Alongside the note type editor redesign, we made a change to how services are configured to a note type. 
  • For myAvatar and myEvolv users, added a check on the save of a note type to ensure that the note type does not include duplicate fields.
  • Enhanced the configuration of note types with custom note sections to show the custom note section type when added to a note type.
  • For myAvatar users, added a check on save of the note type that at least one custom note section long text or a note type section is set to required.
  • Updated the note type editor drag and drop functionality for note type sections.
  • Updated the Import Question display in the Custom Note Section editor to default to the EHR side of the toggle.
  • For myAvatar users, updated Doc Routing handling on the sign note display.
  • Assorted bug fixes. 

Note Type Editor

  • We've redesigned the Note Type Editor display! Since the initial release of Bells, we've been adding to the note type configuration display over time and it was becoming too much for one display to handle. The editor is now organized into labeled tabs for Configurations, Sections and Services.
  • Alongside the note type editor redesign, we made a change to how services are configured to a note type. Moving forward, instead of clicking the menu to configure services, we've added a tab to the note type configuration for the Services to be associated to the note type. 
  • For myAvatar and myEvolv users, added a check on the save of a note type to ensure that the note type does not include duplicate imported question types or signatures across custom note sections. 
  • Enhanced the configuration of note types with custom note sections to show the custom note section type when added to a note type. Previously when added to a note type Bells would show the name of the custom note section and the label “custom note section”, moving forward you will see the custom note section name and the type such as form or narrative. 
  • For myAvatar users, added a check on save of the note type that at least one custom note section long text or a note type section is set to required so the note will always have a place for the narrative to map to. 
  • Updated the note type editor drag and drop functionality for note type sections. The entire card can now be clicked to drag and drop sections from left to right or right to left.

Custom Note Sections

  • Updated the Import Question display to default to the EHR side of the toggle.

Doc Routing

  • For myAvatar users, updated Doc Routing handling on the sign note display to hide the final approval column when the Require Final Approver setting is false. 

 

Session Time Overlap 

  • Corrected a bug in the Session Time Overlap capabilities not catching overlapping notes for different authors. 

 


 


Bells Web App Release - 2024.03.21

Summary

  • We've redesigned the Organization Preference display! 
  • Introducing the ability to configure program overrides for the Session to Sign threshold.
  • Incorporated new configuration for the late note threshold to be configured to include weekends or not. This can be configured across the organization or be program-based. 
  • Added the ability to hold notes within Bells from syncing to the EHR for a period of time. 
  • Updated the Call Masking functionality for the mobile application by introducing a new organization level preference.
  • Added a new validation called Prevent Session Time Overlap. This new validation would prevent notes that have session time overlap with another note from being signed. 
  • For myEvolv users, added the ability to import and configure Sub-Forms in Bells as part of Custom Note Sections. 
  • For myAvatar, users can now have multi-session time increments in standard and group progress notes.
  • For myAvatar, added additional functionality to support future date/time notes in Bells. 

Organization Preferences

  • We've redesigned the Organization Preference display! Since the initial release of Bells, we've been adding to the organization preferences over time and it was becoming too much for one display to handle. The organization preferences are now organized into labeled tabs for Configurations, Compliance, Emails, Appointments (where applicable) and Audit. 
  • As part of the Organization Preference redesign, we're introducing the ability to configure program overrides for the Session to Sign threshold. This will allow organizations to set up separate configurations for programs that allow longer or shorter times for providers to complete their documentation before alerting the note as late. 
  • With the new session to sign time threshold changes, we've added the ability to configure the late note threshold to no longer include weekends in the calculation. This can be configured at the whole organization level or within the program override also being introduced in this release. 
  • Introducing new functionality with configuration in the Organization Preference to hold Bells notes in Bells from syncing to the EHR for a period of time. This can be configured for as little as an hour up to 72 hours to allow processes like maintenance or billing cycles to happen in the EHR without notes from Bells coming in during those timeframes. 
  • Updated the Call Masking functionality for the mobile application by introducing a new organization level preference in the web application to determine if the call masking capability is always on for users at the organization or controlled by the user in the mobile application. 

Validations 

  • Alongside the new tab for Audit in the Organization Preference redesign, we've added a new Validation to Prevent Session Time Overlap. This new validation can be configured to prevent the sign of the note when a session time overlap has occurred and can be configured for specific services.

Custom Note Sections

  • For myEvolv users, added the ability to import and configure Sub-Forms in Bells as part of Custom Note Sections. There is a new custom note section tab for Sub-Forms where sub-forms in Bells can be added and configured. Grid question types can be imported from myEvolv and associated to the sub-form created in Bells.  This update requires myEvolv patch - 11.0.0125.

Progress Notes

  • For myAvatar, users can now have multi-session time increments in standard and group progress notes. When the note uses time increments and depends on the myAvatar form associated to the note type for the note. This update requires myAvatar patch - Avatar CareFabric 2024 Update 16.
  • For myAvatar, added additional functionality to support future date/time notes in Bells. Bells allows cancelled status notes to be signed even when date/time on the note is in the future. This however is not supported in myAvatar so would cause a sync issue. Moving forward, when notes are signed in Bells that have a date/time that are in the future, the note will be held in Bells until after the session date/time has passed so it can successfully sync to myAvatar.

Summary

  • For myEvolv users, added the ability to filter the places of service available when starting the note based on the program for the patient. 
  • Removed the Service Type section in the organization settings section. This was a read-only display and is not needed.

Summary

  • Completed the larger change to the Group note configuration and note flow. This change includes a new Group section under Organization Settings as well as Group Note Type and Practitioner assignment workflows. 
  • As part of Custom Note Sections, introducing a new answer option filter.
  • Enhanced the Large Font functionality to more appropriately increase the font size throughout Bells. 
  • Assorted bug fixes. 

Groups/Group Notes

  • Completed the larger change to the Group note configuration and note flow. There is a new organization setting section for Groups and we're reintroducing the Note Type assignment/practitioner assignment functionality. The new organization setting section shows all active groups for the organization, clicking a group in the display allows you to view the staff and patients in the group, view and manage the note types set for the group as well as view and/or manage the group topics where applicable. When more than one note type set has been configured for the group (where applicable), the user who starts the group note can set what note type configuration will be used per practitioner in the group.  

Progress Notes

  • As part of Custom Note Sections, introducing a new answer option filter to filter out a question's answer option based on another answer selection. This new filter can be found at the bottom of the answer filters via the filter hyperlink on single choice and multi-choice question types. This is done by clicking the Add Dependency Rules button at the bottom of the answer option filters display.  

Application

  • Enhanced the Large Font functionality to more appropriately increase the font size throughout Bells. 

 

Progress Notes

  • For myAvatar users, corrected an issue for notes using SSSM fields causing the note narrative to show in an unexpected place in myAvatar. 
  • Addressed a bug related to filtered out questions being checked preventing the user from signing the note. This was hotfixed to production on 2/23/2024.
  • Corrected a bug as part of narrative type custom note sections not rendering as expected. This was hotfixed to production on 2/27/2024. 
  • Updated validation handling to address an issue incorrectly setting off prompt to change service validations. Updated the prompt to change service types validation type so that Bells will ignore the rule when at least one of the selected services is the service that is being configured, this will make it an “all except” list moving forward. 

Application 

  • Fixed an issue when the user is in dark mode related to the display of text fields in the progress notes.

Summary

  • For myAvatar users, updated the Evidence Based Practice section call to myAvatar to retrieve the strategy codes for the Evidence Based Practice section.

myAvatar Evidence Based Practices

  • For myAvatar users, updated how the Evidence Based Practice section gets the information from myAvatar to include the strategy codes for the Evidence Based Practice progress note section. 

 

Progress Notes

  • Fixed a bug as part of progress notes when the user attempted to copy a scribble to a text section in the note that included list type formatting. 
  • Corrected an issue causing note backups to show when clicking into a patient as part of the Group note workflow. This was hotfixed to production on 2/16/2024.
  • For myAvatar users, updated how programs are filtered to include the practitioner in the filter criteria. 

Note Templates

  • Corrected a bug when creating templates showing incorrect names for the Text sections on the note.

  • Made several cosmetic fixes throughout the application, including Custom Note Section Narratives, Scribble lists, large font displays as well as when set in dark mode.

Summary

  • Enhanced document routing for myAvatar and myEvolv users.

Progress Notes

  • Enhanced document routing for myAvatar and myEvolv users so the sign note display will include an approver search bar when the user has more than 5 approvers available.
  • For myAvatar and myEvolv users, updated the progress note sync for no show and cancelled notes that include a session time section, to no longer display the duration as 0 when synced to the EHR.  
  • Updated the No Show appointment mapping handling for myAvatar users. When an appointment is updated to be no show in myAvatar, the Bells agenda will align with the configured status in organization preferences. 

Groups

  • Enhanced the group note topics for myAvatar and myEvolv users so users who are set as Supervisors can now add/edit group note topics.

Summary

  • Enhanced the new patient signatures for CT|One users.
  • For myAvatar users, updated the treatment plan sync to additionally filter the treatment plans for Bells to prevent duplicate treatment plans from displaying.
  • Introducing a new user placeholder for use in drop-ins, templates and progress notes.
  • Added a new field to the user profile to custom set your Preferred Narrative Name. 
  • For myEvolv users, the No Show status can now be used for group notes in Bells.

CT|One Signatures

  • Enhanced the new patient signatures for CT|One users, Bells patient signatures will now meet the requirement on patient signatures in CT|One. 

myAvatar Treatment Plans

  • For myAvatar users, updated the treatment plan sync to additionally filter the treatment plans for Bells based on the effective and plan end dates. This will reduce the number of duplicate treatment plans/goals users will see in Bells. 

Placeholders

  • Introducing a new user placeholder for use in drop-ins, templates and progress notes. Typing $user will allow you to set the user placeholder to show the signed in user's first and last name. Additionally, added a new field to the user profile to custom set your Preferred Narrative Name. When set the preferred narrative name is what will be used for the user placeholder. 

Groups/Group Notes

  • For myEvolv users, the No Show status can now be used for group notes in Bells. Configuration settings have been updated as well as the group progress note. 
  • As part of a larger change being made to the Group Note configuration flow, the group note type configuration has changed. Instead of assigning note types directly to a group, the note types for the show, no show and cancelled status (where applicable) are saved as a set and applied to a group or several groups at once. This will make group note configuration faster and more efficient when setting the groups up initially. As part of this change, Group configuration has been migrated to the new flow. Bells administrators will need to go into the Group Note Type configuration and name the new sets.

Patient Sync

  • Corrected an issue for CT|One users, where the patient sync to Bells wasn't syncing all patients as expected. This was hotfixed to production on 1/26/24.

Progress Notes

  • Addressed a bug in progress notes that include custom note sections not scrolling the user to the required field within the custom note section when missing on sign of the note. 
  • Fixed a display issue as part of Group Notes when there are inactive patients in the start group note display.

myAvatar Patient Signatures

  • Introducing Patient Signatures for myAvatar users. Patient, guardian or additional signatures can be imported into Custom Note Sections and used in the progress notes. With this feature there is a new view for the Patient, this allows you to set what sections or questions in the note show in the patient view. When signatures are configured for the note, the patient view is shown for the patient to review the necessary information prior to applying their signature. 
    • For more information please refer to the following articles.
    • The corresponding functionality/support has not yet been released on myAvatar. We will communicate once we know what patch(es) are needed to support this functionality. 

Progress Notes

  • For myAvatar and CT|One users, added the ability to limit the note types available based on the selected Program on the note. There is new configuration on the note type to select the appropriate programs to filter the note types when starting a note. 
  • For myAvatar and myEvolv users, added configuration on Note Types to allow you to Override where the Note Narrative maps to for your Bells notes. This will benefit the organizations that do not have a field in their notes in the EHR for the Bells note narrative to go to. 

Application

  • Updated how credentials show throughout the application. 

 

Reports

  • Corrected a label issue in the Signed Note and Sync Issue exported reports. 

Custom Note Sections

  • Fixed a display issue in the Narrative mode custom note section related to how checkbox question types show to the user.  
  • Addressed an issue with the Content question type missing a paragraph break prior to the next question. Added the setting to add a paragraph break to the settings for the Content question type. 

Progress Notes

  • Corrected a bug removing text when utilizing the translate functionality. 
  • Fixed an issue on the print of the progress note cutting signature images on the right side. 

CT|One Sync

  • Addressed an issue causing duplicate session times to show on the note in CT|One when the note was unsigned and resigned in Bells. 
  • Corrected a bug in the patient sync from CT|One when the managing therapist was changed causing the patient to not show in the list for the newly appointed managing therapist. 


 


Bells Web App Release - 2024.01.18

Summary

  • Introducing a new clinical recommendation for Redundant Much?
  • For myAvatar users, enhanced the Missing Information validation to include a new option for “Active Admission Diagnosis”.
  • Updated the Signed and Unsigned Note Report exports and emails to include new columns.
  • Enhanced note type templates to allow more custom note section question types to be templatized.

Clinical Recommendations

  • Introducing a new clinical recommendation for Redundant Much? This new recommendation can be configured to suggest alternative verbs when more than three (3) of the same verb are used in progress notes.

Validations

  • For myAvatar users, enhanced the Missing Information validation to include a new option for “Active Admission Diagnosis”. This new option will allow you to validate the progress note for an active admission diagnosis instead of the standard diagnosis as part of progress notes. 

Reports

  • Updated the Signed Note Report export and email to include new columns for Created Date/Time and Create to Sign Time. The create to sign time will show in two columns, one for hours and a second for minutes. Additionally, updated the Unsigned Note Report export and email with the Created Date/Time.

Note Type Templates

  • Enhanced note type templates to allow more custom note section question types to be templatized. This includes short text, long text and numeric question types. As part of this effort, we've removed the ability to templatize Date and Date/Time question types.  

 

Progress Notes

  • For myAvatar users, corrected a bug causing note types to be offset by 6 hours. This was hotfixed to production on 1/16/24.
  • Fixed a bug preventing users from being able to preview notes after signing. 
  • Corrected an issue when syncing patient's diagnosis. This was hotfixed to production on 1/17/24.

  • Updated the Patient label preference to allow for special characters and spaces. This will allow more than one word to be used to reference patients in Bells. 

 

Placeholders

  • Fixed a bug preventing placeholders from being used in templates. 

Custom Note Sections

  • Corrected a bug from last week's release of Narrative Custom Note Sections that wasn't properly converting the note type when configured as a narrative.

Summary

  • Introducing narrative mode custom note sections.
  • For myAvatar and CT|One users, added the ability to filter to only the services that are available based on configuration for the group. 
  • Enhanced the minimum level selection for the Goals section in progress notes.
  • Assorted bug fixes.  

Custom Note Sections

  • Introducing narrative mode custom note sections. This new type of custom note section allows users to write their progress notes that utilize custom note sections in a narrative way instead of the current form mode. Bells administrators can create custom note sections as narrative or forms. Narrative custom note sections show in an output format, so the user answers within the output and can see the narrative building real-time as they fill out their progress notes. Form custom note sections will follow the traditional question/answer format. 

Group Notes

  • For myAvatar and CT|One users, added the ability to filter the Group Note services to only the services that are available for the note types based on what's configured for the group. There is a new toggle setting on the group note type editor to configure the filter. 

Progress Notes

  • Enhanced the minimum level selection for the Goals section in progress notes to enable/disable checkboxes in the selection display based on the level configured for the note type. 

 

 

Feature

  • Corrected drop-ins to no longer remove line breaks from the drop-in being used when included in the saved configuration.
