EXPLANATION
The LOCUS and CALOCUS forms are available for use within CT|One. As with any form it needs to be included in a packet to be added to a client chart. The current packet can be edited or a new packet created, if a new packet is created make sure to expire the current one to avoid confusion for your staff.

Form
The LOCUS and CALOCUS forms contain all the definitions and narratives for each section and selection. Simply choose from the drop down and check the appropriate checkmarks for each item.

The Results tab will auto calculate based on the selections made in the form.
