Agency logos can be added to the printed report of Form Builder forms and forms from the Form Master. For forms located within the Form Master Maintenance window developer intervention is needed, open a case via the Customer Portal after the logo has been added to the Images Maintenance window.
IMAGE MAINTENANCE
The supported image formats are .jpg and .jpeg. There can be no overlapping of dates for images, an error will present and prevent the record from being saved. The form report will honor the logo that was active on the date the form was originally completed.
Access: Maintenance > Images
1. Open the Images Maintenance window.
2. Open the .jpg image in Paint.
3. Use CTRL-A to select everything.
4. Use CTRL-C to copy everything.
5. Click the Add button on the Images Maintenance window.
6. Click in the Images box of the Main tab.
7. Use CTRL-V to paste.
8. Click the Save button. Note it may take a few minutes for the image to load/present.
9. Click the Edit button.
10. Enter a code into the Code field. The code can be numeric, alpha or alphanumeric.
11. Enter the effective date in the Effective Date field.
12. Check the Report Image checkbox.
a. Prompt presents, click OK to select this image as the form report image.
b. The Code field will grey out and change to “Report Logo”.
13. The Report Image tab presents.
14. Select the default position of the logo; right or left.
15. Select the image shape; square or rectangle.
16. Select the logo size; 3x3, 4x4 or 5x5.
17. Click save.
Form Builder Forms
Saved logos from the Images Maintenance window can be added to any Clinical Form Builder Form.
Access: Maintenance > Clinical Forms > Agency Form Builder > Form Builder
1. Select the form from the form list.
2. Click the Edit button.
3. Open the Report Image tab.
4. Select Yes from the Show Agency Logo on Report drop down.
5. Click Save.
CT|One Forms
Saved logos from the Images Maintenance window can be added to any CT|One form by Remarkable Health development. First add the logo to the Image Maintenance and then enter a case via the Customer Portal. The case needs to include the exact name of the form(s) the logo is to be added to.
