This report provides a visual representation of the client’s first DLA20 score compared to the most recent DLA 20 scores.
Access: Reports > Core Clinical > Client DLA20 Score – 1st Instance to Most Current
Generate Report
1. Enter the report parameters.
Client: Choose the client from the drop down list. The drop down will only display those clients that have a completed Daily Living Activities (DLA) Functional Assessment form in their chart. Type the client’s name or CT|One ID in the Client field to filter the list.
2. Click the View Report button.
