This report provides a visual representation of the client’s DLA 20 domain score, or multiple domain scores, over a period of time.

Access: Reports > Core Clinical > Client DLA20 Domain Scores Over Time

Generate Report

1.     Enter the report parameters.

Client: Choose the client from the drop-down list. The drop-down will only display those clients that have a completed Daily Living Activities (DLA) Functional Assessment form in their chart. Type the client’s name or CT|One ID in the Client field to filter the list.

Start Date: Enter the beginning date of the desired date range for the report.

End Date: This date will default to the current date, enter a different date as needed.

Question Domain(s):  To select domain(s) to be included on the report check the checkbox for the domain from the drop-down list. Click the Select All checkbox to include ALL domains on the report.
 

2.     Click the View Report button.
