Bells & The Patient's Care Team

 

People often ask me how Bells handles a patient's care team. 

First, let's start with “What is a Care Team?” 

Simply put, a Care Team establishes the relationship between the patient and other people involved in their care.  

 

 

 

Here are few things to keep in mind about the Care Team.

  • The Care Team is configured in your EHR
    • CT|One 
      • The Care Team includes anyone marked as part of the Care Team in the Client Status Form.
      • The Care Team includes the Managing Therapist.
      • Parents/Guardians are also included. 
    • myAvatar and myEvolv
      • Shows the staff assigned to the patient in the staff’s caseload. 
      • Caseload for the patient is typically the admitting or attending staff or the staff on the team assignment. 
  • When accessing the Care Team via t@gs, the team also includes the patient's Emergency Contact and Guardian.

 

You can quickly access this information in a few handy spots while you're writing your Progress Notes or Scribbles as well as in the Patient Chart.  

 

Patient Chart

In the Patient Sidebar located on the right side of the patient chart, we show a Care Team section. 

 

Progress Notes

While writing progress notes, you can expand the left hand pane and select the 2nd tab to show the patient's sidebar in the progress note. 

 

Scribbles 

Using the command @ while in a scribble allows you to select from the patient's care team.
