Note Audit Configuration LA

The progress note audit function allows for an option to mandate the auditing or additional review of Progress Notes, by someone other than the original author. During the note audit process, the progress note is not eligible for billing until it is approved by the note auditor. The note audit configuration can be done at the individual staff level or the staff group level.

Individual Staff
Access:  Core > Staff > Staff Master

  1. Open the staff member's Staff Master record.
  2. Open the Facility - Site tab.
  3. Click on the Note Audits sub tab.
  4. Click Add Matrix. 

* Note:  To change an existing Note Audit Matrix record you must expire the current one and enter a new matrix record. Simply editing an existing record will result in progress notes being unassigned for note audit and prevent them from being billed.   


Staff Group

Access:  Core > Staff > Group

  1. Select the Staff Group from the Staff Group maintenance window.
  2. Open the Note Audit tab.
  3. Click Add Matrix.

Note Audit Matrix

  1. Click the Add button to enter a new Review Matrix.
  2. Complete the following fields:
    • Description:  Enter a name to identify the audit configuration.
    • Effective Date:  Enter a date this note audit will begin. This is a calendar date not a date of service, the process will not go backwards to catch previous notes.
    • Auditor Type:  Select who will receive the progress notes for review:
      • Selected Staff - Allows for the selection a single staff member to receive the notes for review. 
      • Staff Supervisor - The staff member's assigned supervisor will receive the notes for review. 
    • Random % Audit: Select the amount of progress notes to be audited per staff member.
      • Yes - The Pct field presents, enter a percentage value.  Note choosing less than 50% could lead to no notes being selected for note audit. The note audit process looks for progress notes every four hours and if a staff member only entered two progress notes in an hour having a percentage less than 50% would result in zero progress notes being sent for review.
      • No - All progress notes written will be sent for note audit review.  
  3. Complete the Service Type tab by selecting Yes or No from the All Service Types dropdown. 
    • Yes - Any progress note that fits the note audit parameters will be sent for review. 
    • No - Only progress notes for the selected service types will be sent for note review. 
      1. Click the Edit List button.
      2. The Selection window presents.
      3. Highlight the service type(s) to be included in the note audit process, move them to the right column by clicking and dragging or using the directional arrows. 
      4. Click OK.    
  4. Click Save on the Matrix record. 

NOTE:  IF the note audit set up is completed on the Staff Group, the individual staff must be added to the Staff Group on the Staff Master.
